50 yo perimenopausal woman with an 18-year history of vaginal changes that have affected her sexually and with intimacy. Since the birth of her first child, she has been bothered by vaginal scarring and anal skin tags. Over the years she has noticed further aging of her vulva and increased undesired cosmetic appearance. Pelvic examination performed with a chaperone present, demonstrated bilateral and asymmetric hypertrophy with hyperpigmentation of the labia minora, excess and redundant bilateral prepuce and ptosis, labia majora deflation and laxity, anal tags, and excess perineal skin. After discussing all surgical and nonsurgical options, she opted to proceed with Excision of Excess Perineal Skin and Excision of Anal Skin Tags. Dr. Aguirre performed the procedures in an in-office surgical setting and at the end of the procedure there was excellent cosmetic appearance of both her perineum and anus. She returned to the office for regularly scheduled postop appointments very happy with her results and healing. She stated she had sex postoperatively with out pain or tearing.
