Case # 22852

55 yo mother of two from Littleton, Colorado was  referred to Board Certified, Fellowship-Trained Urogynecologist and cosmetic surgeon Dr. Oscar Aguirre by her local Primary Care Physician. She has a greater than 10 year history of Lichen Sclerosis Atrophicus which was diagnosed by biopsy. Prior to consulting with Dr. Aguirre her treatment had consisted of application of Trimcinolone Cream, Emu Oil and Castor Oil for comfort. Despite these measures she had noticed severe advancement of her disease. After a thorough examination she was found to have nearly complete agglutination of her vulvar tissues with nearly absent labia minora and closure of the hood over the clitoris. After discussing all surgical and non-surgical options, Dr. Aguirre initiated an aggressive course of Clobetasol therapy. Despite compliance with steroid therapy the Lichen Sclerosis had progressed, at which time she decided to proceed with the recommended surgical procedure. Dr. Aguirre performed, in an in-office surgical setting, Clitoral Un-hooding with Injection of PRP (Platelet Rich Plasma) to the affected Lichen. She returned three months post Un-hooding having noticed improvement on her vulvar symptoms and decreased itching, even without the use of Clobetasol. She presented for her second injection of PRP and returned again at five months post PRP treatment with symptoms and sclerotic changes much improved.
